Form IT-QEE-TP1 Qualified Education Expense Credit Preapproval Form (rev. 10/09)

This form is the first step in applying for the income tax credit for qualified education expenses. The form is filed by the taxpayer and is used to request pre-approval of an intended contribution to a student scholarship organization.

Form IT-QEE-TP2 Qualified Education Expense Credit Computation (rev. 7/09)

This form is the next step in the process of the income tax credit for qualified education expenses. This form is completed by the taxpayer and is attached to their income tax return when it is filed. This form is used to compute the income tax credit for qualified education expenses.
